In respect of our Dementia clinic we are a Golden Ticket practice meaning we are one of several organisations working together to provide seamless care, including primary care, hospital services, councils and voluntary organisations for patients living with dementia. The practice has several appointments available during the week specifically for patients diagnosed with dementia to assist them or their Carer with any specific dementia related issues. We are managing many of our dementia patients in house including review of their medications in order to avoid the need for these patients to have this done in hospital instead. It is our intention as a practice to continue our quest to be become increasingly 'dementia friendly'.
