SPEAK TO MY DOCTOR OR NURSE?
The doctors and practice nurses generally accept telephone calls from 12 noon. The receptionist will ask you to describe the purpose of the call and arrange for the doctor or nurse to phone you back that day.

MAKE AN APPOINTMENT?
Our appointment hotline is open Monday - Friday 8.30am - 6.00pm. We also have an automated phone service available 24 hours, 7 days a week. Telephone 0844 477 3373.
You can use new touch key system without having to wait to:
• Make routine and nursing appointments
• Make 'book ahead' appointments
• Check or cancel appointments
If we are busy you will be put into a call queue - this system will help you to know how busy we are. You can choose to hold or call back later.
Monday mornings are usually very busy - try to avoid calling at this time if you can.
If you have an urgent problem that you think may need a consultation on the same day:
• please provide our trained medical receptionists with information as requested
• one of the doctors or nurses will call you back
• the doctor or nurse will arrange an appointment as appropriate as required OR
• you will receive clinical advice over the telephone OR
• you will be asked to see a nurse for investigative tests or treatment OR
• you will be referred to another clinician as appropriate for your problem.
Urgent, on-the-day appointments are seen by the duty doctor, and not necessarily your clinician of choice. Please make an additional appointment to talk to the doctor or clinician about any other matters of a non-urgent nature.
To be sure of being able to see your choice of clinician, please book a few days in advance. Do book a separate appointment for each person who wishes to be seen.
For all other appointments, advice or test results, please speak to one of our medical receptionists.

HOW DO I...
OBTAIN A HOME VISIT?
Patients are requested to telephone 0844 477 3373 before 10.30am and select the "Emergencies And Home Visits" option. Please give the receptionist as much information as possible so the doctor can assess priority. Home visits are for the housebound or those too ill to attend the surgery. Please remember several patients can be seen at the surgery in the time taken to carry out a home visit.

OBTAIN TEST RESULTS?
Telephone 0844 477 3373 and select the 'Test Results' option. You may call at any time and leave a message or between 1.30 - 3.30pm to speak to the HCA.
Please note that test results can only be given to the patient (or their parent/guardian/known carer) to whom the result relates. Results may be phoned or sent through. Please select the appropriate option when you call.

REGISTER WITH THE PRACTICE?
Please come to the reception desk in person and you will be given a new patient medical appointment at that time. You will need to bring two forms of identification (eg passport, utility bill) one of which must have the address at which you are residing. Patients must reside within the practice area.
You have a right to express your preference of practitioner. When you make an appointment you can select your preferred practitioner. If you require urgent on-the-day care, you will be seen by the duty doctor and not necessarily by your preferred practitioner.
